Private Pay
When primary coverage has been exhausted or denied, personal income and/or savings are used to pay for retirement care.
The daily rate includes a multitude of services, such as room and board and nursing services. However, this rate does not include outpatient services such as therapies (physical, occupational, speech, etc.) and other special services offered by the nursing home. This also does not include any of the monies incurred up to this point from deductibles, coinsurances and/or copays from insurances such as Medicare, Workers Comp, commercial insurance etc.
Each Minnesota nursing facility has 50 rates (sometimes called case-mix or RUGs rates), reflecting the different levels of care needed by residents and the allowable facility costs. If private pay is the original coverage upon admission there will be an Alternate Daily Rate applicable for the first 30 days following admission. Once the care level is determined by DHS, that rate will be dependent on the facilities rates for each care level and will last till the next assessment is due.

Veterans Affairs Contracts
Great Lakes Villa is honored to serve our veterans who have given so much to our country, offering them the best experience in senior living.
We are proud to have contracts with the Department of Veterans Affairs and Community Care Network (VA CCN) in our facilities. While these communities are not federally-operated Veterans Homes, the contracts allow qualifying veterans to receive services through GLV. This contract allows GLV and the local VA to work together by consolidating all community care programs and build an integrated health care network to improve Veterans' access to high-quality care both in VA and in the community.
VA created the VA CCN program to assist Veterans who can't get necessary services from a VA provider either because the services aren't available or the VA provider is too far away. Before a Veteran gets care or services from a VA CCN participating care provider, the VA must issue a referral for an episode of care. VA referrals will include an authorization for a specific standard episode of care. The referral will include a start date and an end date, along with a specified number of visits and/or services. GLV will work directly with your provider and CCN case manager to obtain prior authorization for the stay.
Insurance
Knowing your treatment is going to be covered by insurance can relieve stress for residents and families, and will help in the recovery as well. We will help families maximize their benefits whenever possible as our insurance experts are the best in the business at finding the most benefits from each individual's coverage plan. The GLV team will work diligently ensuring all appropriate prior authorizations are obtained if necessary.
We are contracted with many of the nationally recognized Medicare Advantage plans, Minnesota Department of Health's managed plans, and manu third party insurances such as long term care insurance.
